Thanks! You're right. I've changed it to a unmanaged gateway, but the
result is the same.
When I specify a caller id number on the gateway then it works, but I
want to define it per user.

What do you mean by "creating a SIP trunk"?  The Teles is a gateway -
you should just configure it as a gateway.
