 This can be done with Polycom phones...

User line>Diversion>Forwarding ENABLED (push profile).

At that point a new idle icon is available on the phone. You simply push it and 
key in the line where you want the calls to go.

As far as I understand it, it is phone specific. What type of phone do you use?
