I did load from the 4.0.4 ISO.

I ran into something weird installing 4.0.4 from ISO... Iptables loaded.
Did
you check to make sure iptables is not running on your server?

Did it also get the same host name and sip domain name?

If not, the certificates you restored with the backup are wrong...
