HD only works when calling another HD handset. When you call another HD handset and there is nothing blocking the codec from being passed or negotiated, it will work.
For Example: Calling an internal subscriber locally will pass HD. Calling via the PSTN will likely not work as most ITSP's won't allow the protocol at this time.
