It hasn't been updated since April 2nd and many users have been added via
the GUI and can be seen in the Postgres DB.   I'm running version
4.0.3-016870 2009-10-16T21:43:58.

What is changing that makes you think it needs to be updated?

What version are you running?

 

Additionally, 4.0.3 was not a stable release, but an unstable release if I'm
not mistaken.   Stable release was 4.0, 4.0.2, and then 4.0.4.   Hard
telling what might be going on without working on a stable release, and the
unstable releases are typically not supported in the sipxusers group.
