Contrary to what the carrier initially said, they will only accept inbound
calls on port 5060.

We can register on port 5080 and they will send inbound calls there. We can
now send calls to them on port 5060 and this works also.

What information would you need in order to make a template? I used the
voip.ms template, and simply made sure the p-asserted-identity was the main
10 digit phone number.
