The timer should be kicking in to cleanup these calls. It has been an
ongoing issue since 3.10.x.

I think they time out, I'm sure there is a way you can set the timeout
to shorter if you lilke.

As far as where they come from, a SIP dialog that end abruptly due to
a phone crash, network drop whatever.

These call are more of an accounting thing, not a resource consuming
thing AFAIK.
