I think I was one of the "bad power supply" issue Tony describes. It isn't the same, but I'll recap for anyone as I have some new info that may be of use to some.
Periodically we would hear a loud crackling noise (nothing made my blood pressure shoot up faster). Upon inspection the power supply module was fried. You could see black scorch marks on part of it where a power transistor exploded. Eventually we figured out that this happened every time we opened the drop ceiling. I then guessed what was happening. In our data center we vented our AC unit into the plenum. I believe it charged (ionized) the air which made its resistance drop substantially so that it would arc over the power supply. Seemed kind of wild, but since I had nothing else to go on we overhauled our Datacenter. The drop ceiling was removed and a drywall one was installed further up. Cable trays were used for wire instead of the ceiling space. The AC was vented to the exterior. Since then, no blow ups. So if you are having the issue, look for something that might be charging the air around the unit. As for the MP 114's thanks for the heads up. I haven't flashed any of our to 6.0 yet, but if we have issues I will post here.
