I have a question for users like Chuck Reichel and others with visual  
impairments. I was wondering if it would make sense to add voice-over  
support for notes and links in the PDF. Currently you can step through  
notes and links using option-tab and shift-option-tab. But voice-over  
doesn't notice this. I could imagine this to be useful for you though.

One problem is that the note or link will steal the focus away from  
the main text, so voice-over wouldn't read the main text. Currently  
the only way to get rid of the focus on the note or link is using a  
mouse click, but perhaps that would not be too  easy? Perhaps there is  
a natural shortcut for removing focus from an element? What would you  
do in a browser, where I know links support voice over?
