I haven't seen anything like this. Perhaps your launch services database is 
corrupted. Try running the following line in Terminal:

/System/Library/Frameworks/CoreServices.framework/Versions/A/Frameworks/LaunchServices.framework/Versions/A/Support/lsregister
 -kill -r -all local,user,system
