It does seem to behave differently. If I enter "risk factor" without quotes in 
Preview, I get all instances of "risk" and "factor" [OR search]. If I enter 
"risk factor" with quotes, I get only the instances where risk and factor are 
together [AND search].

In Skim,  If I enter "risk factor" without quotes, I get all instances of 
"risk" and "factor". If enter with quotes, I get nothing. If I enter "risk 
factor, I get all instances of factor. If I enter risk factor", I get all 
instances of risk.  Skim seems to be looking literally for "risk or factor".
