This has been answered before on this list.
Note are added to a page, not to the text of the page. So they can also only be
ordered according to their position on the page, and not according to their
position relative to the text. That information is simply not available, and
moreover also the information about how the text is ordered (like columns) is
not available.
There does not exist any nesting or in other way linking between notes. And we
cannot show something that does not exist.
And I am talking about the facts about how the PDF format is organized. This is
not a choice we make.
