When this happened to me, the pull-pulls for my rudder were not tight
enough.  The high velocity of the throw would blow the rudder back resulting
in a sickeningly graceful rainbow arch that was terminal.  Sometimes I could
pull out at the last second, sometimes not.  Tightened the pull-pulls and
the issue went away.
