Check the archives for various postings about postfix and exchange
servers, or search for amavisd-new or mailscanner(?). Basically,
most people run a helper program such as amavisd-new or mailscanner
that calls spamassassin, as well as any virus scanners, then does
what you want with the mail (reject, discard, quarantine, etc.).
