My work email address has it going out as Gary W. Smith as we have 10+ Gary 
Smiths' in the organization.  Some people use it because they can.
 
Maybe this rule in conjunction with others might make sense under a certain 
condition but it probably isn't that praticle for business use.

It seems pretty common to me.  I have quite a few examples in
my inbox right now, from five different people within the last
few days (one doesn't use a period after the middle initial). 
And of course that's the "From:" line I normally use.
