Probably a DNS glitch: slow response (SA doesn't wait forever), dropped
packets, etc.

Sounds like slow response.  The second time, the result is in the cache.

Just look for RCVD_IN_* in your caught spam folder.  If you never see
it, then network rules are disabled for your site.  I have no idea what
MIMEdefang does by default.
