There are two things repeatedly mentioned on this list with the Outlook toolbar on recent versions of Windows.  As Skip mentioned, you must install in a location other than the default; C:\spambayes is usually recommended.  The other thing is that the Spambayes toolbar will not work in a 64-bit version of Outlook; it must be 32-bit.  Being a dormant project, if you have met these criteria and it still does not work, then there are no further options other than to search Google's archives for a solution, use Spambayes POP3 proxy instead, or find an alternative software.
