[SC-Help] When to appeal?

The FAQ isn't entirely clear as to when an appeal is appropriate.  I have
on several occasions received an indication that the ISP involved has
taken action, yet when I go to the web site advertised in the SPAM, it
is still active.  Does this warrant an appeal?  What does "action" mean
in action already taken?
