Also check VLANs/VRFs and TFTP source interfaces. For example, if you're polling on a dedicated management IP, make sure "ip tftp source-interface" is set to that particular management VLAN/Interface. You may already have had to set some devices up with specific trap source interfaces for SNMP - it's no different doing the same for TFTP.
On devices that are vrf aware, you may need to use the vrf name as well.
