I did a retrofit installation for a rock climbing storefront in Raleigh back in the day. We ended up having to put sprinkler heads in any overhangs over 4' deep because the AHJ demanded it. We used Tyco RF-IIs and the tenant put huge warning signs above the sprinkler heads letting people know that they weren't handholds. As far as I know they didn't have any problems.
So you may want to contact the AHJ and find out what they're looking for and willing to accept as far as protecting and 'caves' goes.
