oh, I though you were asking how to use psycopg's API.   If you want to use 
SQLAlchemy techniques, you would use TypeDecorator:

your type would look just like 
https://docs.sqlalchemy.org/en/14/core/custom_types.html#coercing-encoded-strings-to-unicode:

from sqlalchemy.types import TypeDecorator, Integer

class CoerceNumpyInt(TypeDecorator):
    impl = Integer
    cache_ok = True

    def process_bind_param(self, value, dialect):
        if value is not None:
            value = int(value)
        return value


that would be it
