What do you have in the "Tag" column in the track listing in MP3tag? If you have more than one tag type listed, for example both ID3v1 and ID3v2, this can cause the problems you describe.
To fix this, go into menu option: tools > options > tags > mpeg and set the checkboxes to match the settings shown below. The next step seems a bit extreme, so make sure you have a backup of the source files before executing this! Select all tracks. Click on the red X (Remove tag) and them immediately click on the Undo arrow just to the right of the X.
