What you are asking is essentially the same as connecting a client to a
remote server over the internet.

Pippin talks about doing that with iPeng here
-http://penguinlovesmusic.de/ipeng-the-iphone-skin-for-squeezecenter/how-to-use-the-ipeng-application/known-issues/coolios-guide-to-remote-playback/

Over the internet this is NOT advised as your LMS becomes open to
hackers and instead you are advised to use a VPN. However the principle
for what you want to do is as he states - TCP ports 9000 and 3483 and
UDP ports 3483 need to be opened from your client subnet to your server
subnet. It is highly unlikely that you will be able to do this using a
domestic router.

Please explain the components in use and why you are running on two
subnets as described.
