That log entry should contain:
Request(minute/blacklist/-) http://whatever yourip...

That log entry shows that you are being identified with the minute 
source, and have been redirected by the blacklist destination.

If that is correct, then after restarting 'squid -k reconfigure', if
you keep hitting enter within 60 seconds, you should be processed 
by the default rule after the time expires.

Your default rule says pass none and 
redirect http://www.altuwairqi.com.sa
so that's what should happen.

It worked for me using 300 seconds.
