You have two choices: firstly, try and make your modifications into
plugins, which are far easier to maintain separately from the main code;
secondly, modify the main source in as few locations as possible and
make patches.  My experience is that patches can often be applied to
later versions of the code with no modification.

You may need a combined stratetgy: most of your code in plugins with
some small patches to help integrate them (e.g. provide the right
hooks).
