Browsing through the Courier-IMAP Changelog at
http://www.courier-mta.org/imap/changelog.html shows many references to
Unicode and language related fixes between your 2.2.1 and the current
3.0.8.  It may be that your old version of Courier-IMAP is presenting the
header wrong and Squirrelmail is confused while Outlook figures it out.
Just a guess.
