Problems with Openssh

I have read all the documentation and tryed to get it to work.
I am trying to install openssh on Redhat 5.2 and 6.2  I have the
same problem with both.  I have coped the pam file over to the 
/etc/pam.d directoy and have started the sshd system.  When I 
try to log in, I get denyed.  My log file shows that pam is 
not able to authenticate the user.

What am I missing?
