Jerry I used a SM4200DT when I had similar and different problems twice, and I believe the one like yours was solved by updating my browser to a higher version - probably 4.79. I now have upgraded to a G3 card.
Also make sure you don't have that infamous Objectsupportlib installed - I still find it gets reinstalled every once-in-awhile by some software including fairly recent Mac operating systems. It can screw you up but good. Good Luck.
