Just occurred to me: Libs are not actually files to my knowledge, but
sections of the Mac OS ToolBox (API), like AppearanceLib and InterfaceLib.
There won't be a file of the name OTUtilityLib, but presumably that part of
the API will be provided by one of the Open Transport extensions or shared
libraries.

Quite why I'm seeing people suggesting turning off Open Transport is beyond
me - Open Transport is the Mac OS TCP/IP software needed for networking and
Internet connections. Disabling it will be pointless, surely?

Not sure what the real problem is, though, I've never had that complaint.
Especially odd after an OT re-install. "My root cause of the problem seems
to be that if I turn on the "Shared Library Manager PPC"...". Well, for one
thing, I think you'd need Shared Library Manager to access any OT shared
libraries (which are files containing re-usable code made available to
applications). As for why that itself is failing, I'm not sure. The 68k
Shared Library Manager isn't turned on, is it, because I don't think its
meant to be.

As David Klaus suggested, try looking for extension conflicts.
