Extension management is a bit tricky, as you cannot guarantee to know what any of them really do, and whether extensions you want to keep have dependencies on any others.
However, something you can nevertheless try is to go through all the extensions in the Extensions Manager control panel, and see if there are any that you don't need. Disabling Folder Actions is probably a good thing to do (unless you use it) as I found it slowed my Finder down quite a bit. At the bottom of the control panel, click the disclosure triangle to reveal the info pane, and in there you can get a description of many extensions, to help identify those that serve you no purpose. Be very wary about disabling those that you don't know what they do, they might be very important. This should, at least, save some RAM, and may afford a slight speed increase. It will, though, decrease your start-up time :) As for the differing extension counts - not all files inside the Extensions folder are system extensions - those in, for example, the Modem Scripts, Global Guide Files, and PPP Connect Scripts folders inside the Extensions folder are not loaded at start-up.
