I had the same problem in OS8.6 with a Kensinton USB optical mouse. It
turned out that, when plugged directly into the USB card, the mouse was
getting powered up (its red taillight came on) soon after my Max started up
but before any extensions loaded, including the mouse driver extension. For
some reason, when that happened, the driver would not see the mouse until I
rebooted. I then tried a powered USB hub between the mouse and the USB
card...same problem.
Then I unplugged the power to the hub, and the problem went away: The mouse
did not receive power until the extensions loaded, and it functioned
properly at the first boot-up.
