In section 4.1, please change:

The IV shall be exactly 12-byte long.

to:

The IV shall be at least 12 bytes long.

to be consistent with the GCM specification that the IV can be longer. (See section 4

and see the McGrew/Viega GCM paper secion 2.1, p 2).
