Having discovered the raidctl utility (finally!!!), it appears that the
controllers are behind on firmware version:

lo...@dev-storage-01:/# raidctl -l 4
Controller      Type            Version

Current version on LSI's site is 1.29.0.0.

Does this sound like the cause of the issue I'm seeing on both of these
machines?  Anyone with specific recollections about this level of
firmware?

Secondarily, it appears that I can update the firmware using the raidctl
utility for these controller cards.  Can someone  confirm the procedure?
(Is single-user mode required, or quiesce the controllers somehow?)
