Apple's site has become so annoying. If you can't find a copy, I can email you one (hopes apple lawyers aren't watching). It's about 1.4mb in size and you need a MacOS computer and a copy of Diskcopy to actually make the floppy that will boot the machine. The only utils on it are drive set-up and disk first aid, but it should let you determine if the HD is the culprit.
