Based on your answer, it sounds like your SCSI drive may have some age on it.
My experience has been drives will eventually get to the point of needing replacement and no amount of reformatting and running DiskWarrior will help.
If you drive is the original SCSI that came with the unit, I'd say it's time to punt it or use it as an occasional holding tank.


Although my s900 is semi-retired, the best thing I ever did for it was to install an IDE card and 7200 rpm ATA 100 drive, which is very inexpensive and basically plug-and-play even if you're still operating OS-9.
(Mine is kept as a 9.1 machine.)


It's just a suggestion, and the ATA drive can be used as your startup disk with no problems.

If you do add these, the DIMM chips, drive ribbons, and most stuff that unplugs can be reseated for good measure.

These are the only non-technical suggestions I can make before you dive into more complicated diagnosis others may chime in with.
