I'm guessing that this is your problem. I've got 37 jacks in the house that all go to patch panels in the hall closet. When I did the installation, the failure rate of punched connections was significant (somewhere around 5%). I found it necessary to check continuity between the patch panel and the jacks using an ohmmeter to make sure that each connection was good. Of course, this required a long bit of wire as well.
There are of course tools available to quickly diagnose whether or not you've got a good connection - starting from about $50 for one with lights that just shows good/not good, to those that show you when you've accidentally crossed pairs, on up to very expensive stuff. If you know someone who does a fair bit of network wiring, they probably at least have the cheaper version. I've found it indispensible.
