I see now. MathJax requires two dollar signs ($$x^2$$), whereas the math_dollar/pngmath requires one dollar sign ($x^2$).

I see now that the configuration is set in the mathjax.py file. So I'll try to figure out how to fix it. Maybe I can also use this to add the preamble.

By the way, from what I can tell, \middle comes from e-TeX. It isn't listed at http://www.mathjax.org/docs/1.1/tex.html#supported-latex-commands. I asked about this on the MathJax user group at https://groups.google.com/d/topic/mathjax-users/RYI3f239J3I/discussion.
