No, I don't think so. UTF-8 is the default for vCard 3.0 and iCalendar
2.0, regardless of what the encapsulating XML says.

Can you reduce it to a simpler combination of syncs, say, add special
character in FB, send to server, then to phone?

Check at each step that the data was transfered okay. The data stored by
the file backend uses UTF-8.

Regarding the duplicates, does that occur during a normal two-way sync?
If in doubt, identify one sync which shouldn't create duplicates but
does and post the server-side sync log, either publicly or to me
directly.

Unfortunately there isn't much that can be done about this. Picking the
subset of the data which is supported by all devices typically works
best.
