Stuart, wow in the 90's, you are old ;-> What you suggest is very commonly done. In fact you don't even need to go to r.php to do it, you can read and interpret it on the destination page itself. What you do is add to your rss link a source: http://www.example.com/yourpage.php?source=rss_feed. Now the problem is that others may simply forward on that link, so you don't know for sure if the hit was from your rss feed, or from someone passing on your rss feed link. Arguably it is the same.
We have a counter over at http://htmlfixit.com/free.php that will keep track of many things, including pages hit and referrers that could be used for a more rudimentary tracking. We offer a free version and a $10 version for the big spenders :-) We have also written some code in the past that does just what you suggest both for use in your r.php?id= setting and in the direct read from the target page setting.
