I have used TB IMAP4 to get mail from an M$ Exchange 5.5 Server, and only gave it the mailbox name and password. Have you tried it? Are your mailbox and login ID's different? Domain/AD? What ver of Exch? Did you try POP3 (you may not like TB's current IMAP implementation, I haven't found it to be better than POP...)? Have you asked your Exch Admin's how to IMAP or POP you mail?
