Are these outbox folders configured to "Compress on exit"? I'll bet
they are uncompressed and occupy many megabytes, all of which are
traversed, one deleted (sent) message at a time, every time a new
outbound message is saved to it.

That's my guess. Compress the outbox folders and see what happens.
