Two way is hard in general.

For our twit.tv podcasts, Leo Laporte has resorted to using independent
Skype calls to each of the participants, mixing them with a video/audio
mixer at the TWiT Cottage in Petaluma (CA), and sending the mix back
upstream and to the distribution channels.  It generally works, but if
someone with a long delay to Petaluma (Europe or Australia) is on the
call, you have to adopt some protocol for who might be allowed to speak
at any given moment.
