This sort of thing has been discussed many times - and to some extent, 
achievements have and are being made.
However, it's a very complex problem, in part because the TiddlyWiki 
universe is in many ways very different from most extensible 
applications/systems (for example, a centralized distribution system is 
not as easy to implement, and possibly even not as desirable, as it is 
in Mozilla's landscape).
Nevertheless, there will certainly be further advancements - though of 
course the amount of developers available to work on a given project is 
frequently the bottleneck.

FWIW, the existing plugin manager (backstage > plugins) already provides 
some functionality for handling plugins.
