This has happened to me on both Windows 8 and OS X 10.7.5, both using Firefox (latest version, 26), and has happened to three separate .html files I've been working on. Would my editing $:/core/ui/SideBar/Tools to include a new button have any effect on this? Let me know if you wish me to send the "empty.html" file.
