I have no experience with school-wide online evaluations, but I have tried
to get students taking an online course to do an online evaluation.  The
problem I had was low participation.  I had set it up as an anonymous form.
Seems that students who did not want to do the evaluation felt secure in
knowing that they would not be identified.  In a classroom setting there is
social pressure to fill out a questionnaire that is distributed to all
students.  Unless you have some password protected system, there would also
be the chance of multiple submissions.
