I realize now that this is actually the same thread as the previous 
one about sending a final bounce message when someone is dropped 
from the pending queue (although we have not yet started quoting RFC's :-).

I don't have (interesting) statistics for how many senders fail to
confirm yet, but I suppose I will after watching my logs for a while.

For me, the underlying issue is whether I can reach a stage of comfort
that allows me never to look at the tmda-pending queue. Maybe this is
unrealistic, but it seems as long as I have to sort through the tmda-pending
list, I have not really advanced that far from sorting into a spam folder, 
albeit with a more reliable sorting mechanism.

I am new to using TMDA, so I can't even claim the authority of 
experience.  Maybe the issue of people who don't confirm is not that big 
of a deal.  I think  most people are not in the same situation
as you (Jason) with respect to confirms. I personally never get mail from
a real person that I want the mail delivery agent to delete, i.e. I would 
never send someone a confirm message that said "Please don't respond if...".
I can see how some people might need to shelter their mailbox from real 
people as well as spam, but I just don't have that many fans :-)
