What is the filter rule that matches (the one that is recorded in
LOGFILE_INCOMING)?  Is it a 'to-file' or 'from-file' line?  Also, if
you want to send the whitelist to me off-line rather than posting it
publically, I can look at it to determine if there's anything
suspicious in it.

Nope.  TMDA pretty much "works as advertised".  If a message is
getting through, then there is a reason it's getting through.  I don't
see anything unusual in the header you posted.  The sender isn't faked
and it appears that the recipient was '[EMAIL PROTECTED]', which
matches the To field.
