Here's something that happens to me: I have an account at mailsnare.net that forwards to my home server. Sadly, they use my mailing address at mailsnare.net as their Return-path header, even though that is explicitly the Wrong Thing to do. Consequently, if I choose the "whitelist" option in tmda-pending, the Return-path is whitelisted, and suddenly spam from that account starts coming through.
