As far as your challenges sticking in queue,
there is a limit of outbound requests for confirmation (you can set it to you want) so your outbound queue does not fill up.
If someone is really bad they would send from different addresses and each would request a confirmation.
I would then plan on limitations using other methods (block by IP and tarpit).
TMDA or not, if your system is mis-configured a spammer, or someone with other intentions, can crash your box.
