release of confirmed mails question

I am new to tmda and I have it working on osx panther server with its 
default setup of postfix and cyrus imap.  What I have been noticing is 
that when a new message comes a message goes back to the the originator 
to confirm and then the originator can confirm.  When that is all done 
and the originator sends another message, the second message goes right 
through, but the first message will not go through until another 
unconfirmed e-mail sends a message in.  Why does it take an 
unknown/unconfirmed e-mail to come in to trigger the first message to 
get delivered.  After the confirm all the other e-mails seem to go 
fine...
